These claim forms (formerly HCFA-1500) are used to submit health insurance claims for services provided by health care professionals to Medicare and Medicaid, insurance companies or clearinghouses for processing. Printed in red ink on special bond paper in strict compliance with government regulations and specifications. Available in both laser and pin-fed continuous formats, with or without a Bar Code.

UB-92:
These hospital claim forms are used to bill health insurance claims for services provided by hospitals to Medicare and Medicaid, insurance companies or clearinghouses for processing. Forms are printed in red ink on special bond paper in strict compliance with government regulations and specifications.

ADA Dental Claim Form:
Authorized by the American Dental Association, this form is used by dentists to file claims with insurance companies on behalf of their patients.

The National Uniform Claim Committee (NUCC) has released its semi-annual updates to the Health Care Provider Taxonomy code set, which will go into effective on April 1, 2010. The complete code set, including lists of new codes added and modifications made, is available at: www.nucc.org/taxonomy. The PDF download version of the code set will be available shortly. The changes to the code set include the addition of a new code and addition of definitions to existing codes. When reviewing the Health Care Provider Taxonomy code set online, revisions made since the last release can be identified by the color code; new items are green, modified items are orange, and inactive items are red. The Health Care Provider Taxonomy code set is maintained by the NUCC. The NUCC is a voluntary organization of health care industry stakeholders representing providers, payers, designated standards maintenance organizations, public health organizations, and vendors.
